First time.....
Today was the first time after 25k miles of being tuned that my diverter valve popped off. It was during a run in 2nd gear from 30 mph. After the run, a coupe of miles later I had no boost when I got on it, all I heard were the turbos spoiling. I was like WTF? My wife was nervous as she was in the passenger seat. I pulled over popped the hood, I was relieved when it just turned out to be the diverter valve popped off. Popped it back in place and went on my marry way.

I had that happen to me once, it's quite unsettling to hear that noise coming from the engine bay. I popped the hood and was shocked to see that the DV wasn't locked (neither of them) which is why it popped off. I locked them and haven't had a problem since. I'm still waiting because I know that since I'm tuned there's a good chance they'll pop off again.

No issues with my stock DVs when I had GIAC and now that I have Cobb AP. I upgraded to Forge DVs because I got a really good deal on them and also because I suspected that the stock DVs are causing boost fluctuations...
