Mfactory Core Return Address??

Since Mfactory is currently being acquired (I think) and has been super slow to respond to emails, does anyone know the return address for differential cores?

I heard back via email and they sent me a link to a page with a Form but I couldn't find it anywhere (on mobile)

Anyone else send their core into Mfactory? Was there a form to fill out or did you just mail it to them?

Lol I tried Google Chrome from my work computer and the form showed up fine... Weird. Shipped today, filling out the form now.

Yea i also informed them about the form not working on mobile. They said they would notify their web designer to fix it. That was in february. Lol
