i've used hagerty and ontrack. thankfully never had to file a claim so don't know who's better there, but ontrack was substantially cheaper.

It's just based on agreed upon value, your SLIP rating, car modifications, etc.
You can put in whatever coverage amount you want. With ontrackinsurance the last time I purchased it for two days, it was $5.72 per 1,000 of coverage.
